Middlesex Healthcare Services offers home-based care options, including Adult Foster Care (AFC) and Skilled Nursing Home Care, designed to support both individuals and caregivers. Each program provides a different level of support depending on care needs.

Skilled Nursing Services

Professional nursing services provided in the comfort of your home

We provide clinician-guided medical and nursing services in the comfort of home.

Our core Skilled Nursing services include:

Clinical Care:

Acute and chronic diagnosis management, surgical aftercare, and wound/ostomy care.

Diagnosis management:

Acute and chronic diagnosis management, surgical aftercare, and wound/ostomy care.

Specialized Nursing:

Medication instruction, pain and symptom management, and psychiatric/substance abuse support.

Home Health Aides (HHA):

Provide essential personal care and basic health monitoring for patients at home, assisting with (ADLs) like bathing, dressing, grooming, toileting, and mobility.

Home Therapy:

through our collaboration with other home team members, we provide: Physical Therapy (PT), Occupational Therapy (OT), and Speech Therapy (SLP).

Adult Foster Care

Home-based daily assistance supported through a state-funded program, with guidance for caregivers and structured care plans.

What is AFC:

The Adult Family Care (AFC) program in Massachusetts is a MassHealth-funded initiative that provides a tax-free stipend to caregivers (family or non-family) who live with and support individuals aged 16+ with disabilities or chronic illnesses who cannot live alone. It serves as an alternative to nursing home care, with caregivers receiving training, support, and a monthly stipend, while participants receive 24/7 care.

Core Requirements:

  • Caregivers must be at least 18 years of age and demonstrate the responsibility and capability to make precise decisions. Must not be a spouse, and not the legal guardian.
  • Participant/Patient: Individuals must be 16+, have a medical/mental diagnosis requiring daily supervision, and be eligible for MassHealth.

Caregiver Support:

A team consisting of a nurse and a case manager conducts home visits to provide training, oversight, and ongoing support.

Financial Compensation:

Caregivers may receive a tax-free stipend ranging from approximately $9,000 to over $21,000 annually, depending on the level of care level.
