Middlesex Healthcare Services Adult Foster Care offers specialized assistance to people with medical, physical, and/or cognitive conditions.
Our community-based Adult Foster Care Program is licensed by the state of Massachusetts. We provide our care to individuals who are 16 years old or older.
What is Adult Foster Care?
Adult Foster Care (AFC) is a state-funded program that transforms the lives of seniors or individuals with disabilities by providing living alternatives. The program provides professional assistance in daily living activities or ADLs and instrumental activities of daily living or IADLs.
ADLs and IADLs are daily life tasks that are essential for a quality life. ADLs refer to basic tasks necessary for independent living while IADLs are more complex tasks that are incidental to a person’s care.
How does AFC work?
Middlesex Healthcare Services works together with an expert team of RNs, Case Managers, and other essential healthcare providers to deliver care, provide home visits, monitor health needs, and develop an individualized plan of care.
